Sagging is the change patients find hardest to describe and easiest to feel. The face has not gained lines so much as lost its outline, and the usual way of putting it is looking tired, or heavier, or simply less like yourself. What is happening underneath is more specific than it feels.
SmileLift™ and SweatZap™ were both developed by Sam Assassa, MD, who founded Beverly Hills Aesthetics in 1995. He also founded the American Association of Aesthetic Medicine and has served as its President since 2006. Dr. Assassa received his medical degree with honors from Damascus University, completed his internship at Loyola University affiliated hospitals in Chicago and his residency at Case Western Reserve University affiliated hospitals in Cleveland, and is board certified in Internal Medicine.
Deflation, Descent, and Laxity
Three processes produce the same impression and they need different treatment.
Deflation is volume loss. Fat pads shrink and bone resorbs, so the face flattens and the structures that gave it shape diminish. This responds to restoring volume.
Descent is movement. The volume is still there but has slid downward, collecting where it did not used to be. Adding more volume to a face that has descended enlarges it rather than lifting it, which is the mechanism behind results people recognise as overdone.
Laxity is the skin itself losing elasticity, so it no longer recoils and begins to drape rather than fit.
Most people have all three by their fifties. Which predominates determines whether a plan is built around restoring, repositioning, or firming, and getting that wrong is why some patients feel their treatment made them look fuller rather than younger.
Where Sagging Shows First
Where the concern is lines rather than loss of firmness, that is covered on wrinkles.
The Limit Worth Knowing
Non-surgical treatment handles early to moderate change well and genuinely so. Beyond that range it improves appearance without resolving the problem, because tissue that has descended substantially needs repositioning and excess skin needs removing, and no injectable or energy treatment does either.
A patient with significant laxity treated with filler ends up with a fuller face rather than a lifted one. Being told that before you spend anything is worth more than a series of treatments that were never going to deliver it.
