Forehead Lines

Horizontal forehead lines carry a complication most facial lines do not. The muscle that creates them is also the only muscle lifting your eyebrows, so relaxing it enough to smooth the lines reduces how much it lifts. That tension is the whole story of treating this area, and it explains the most common complaint patients bring from a previous injector.

What Causes Forehead Lines

The frontalis is a broad flat muscle across the forehead, and its only action is raising the brows. Every time you look surprised, raise your eyebrows for emphasis, or lift your brows to open your eyes wider, it contracts and the skin above folds.

Over decades that folding etches horizontal lines. They deepen faster in people who are naturally expressive, and faster still in people who unconsciously raise their brows to compensate for heaviness in the upper eyelid, which becomes a cycle.

Sun damage and collagen loss deepen them further by reducing the skin’s ability to recover from each fold.

Why the Brow Drops

This is worth understanding before any treatment.

Several small muscles pull the brow downward and the frontalis pulls it up. Brow position is the balance between them. If the frontalis is relaxed broadly to smooth forehead lines, the downward muscles are unopposed and the brow settles lower, producing a heavy or hooded feeling.

The lines are gone and the eyes feel smaller. That is not a product failure, it is a placement decision, and it is avoidable.

Patients who compensate for existing brow heaviness by holding their brows up are the most likely to notice it, because the frontalis was doing more work than they realised.

Lines at Rest Versus Lines in Motion

If your forehead lines only appear when you raise your brows, relaxing the muscle prevents them deepening and they largely disappear.

If they are visible with your face completely still, the skin has creased and muscle treatment softens them without erasing them. Those need the skin treated as well.

How Forehead Lines Are Treated
