Why Hands Age Faster Than the Face

Three things work against them.

Very little subcutaneous fat to begin with. The back of the hand has a thin fat layer, so losing a small amount produces a visible change. As it thins, the tendons, veins, and bones beneath become prominent, which is the skeletal look patients describe.

Thin skin that gets thinner. Skin on the back of the hand is among the thinnest on the body and it loses collagen the same way facial skin does, becoming crepey and translucent.

Constant sun exposure with no protection. Hands are exposed every day, on the steering wheel, at a desk near a window, outdoors, and almost nobody applies sunscreen to them. Age spots appear here earlier and more densely than almost anywhere.

Three Separate Concerns

Worth separating, because they need different treatment and patients usually have all three.

Volume loss produces the hollowed, tendon-prominent appearance. This is what filler addresses.

Pigmentation produces age spots and uneven tone. Filler does nothing for it, and it responds to resurfacing and chemical peels.

Skin quality produces the crepey, thin texture. Skin boosters and microneedling address it.

Treating volume alone on a hand with significant pigmentation produces a fuller hand covered in spots, which is not the result anyone wanted.

Filler Versus Fat Transfer

Both restore volume to the back of the hand and they suit different patients.
