Botox® for Jaw Slimming

A wide lower face is usually assumed to be bone structure or weight, and in a significant number of patients it is neither. The masseter is the chewing muscle running along the outer jaw, and like any muscle it enlarges with use. Years of clenching, grinding, or heavy gum chewing build it up in exactly the way repeated exercise builds any other muscle, and the result is a jaw that reads as square rather than tapered.

Where that is the cause, it can be reversed. Where the width is skeletal, it cannot, and knowing which you have is the entire consultation.

How Botox® Treats a Square Jaw

Injecting a neuromodulator into the masseter reduces how forcefully it can contract. A muscle that contracts less loses bulk over time, the same way an unused muscle atrophies, and as it reduces the lower face narrows into a more tapered shape.

The mechanism is gradual by nature. There is no immediate change, and patients expecting one will be disappointed at their two week check. Narrowing typically becomes visible at four to six weeks and continues developing across repeated treatments over months.

Please note: masseter reduction and facial slimming are off-label applications. These products carry FDA approval for indications that do not include jaw contouring. Off-label use of an approved medication is legal and common in medicine and is discussed at your consultation.

Determining Whether You Are a Candidate

Candidacy is established by examination rather than by photograph. Dr. Assassa will ask you to bite down while palpating the outer jaw. A masseter that feels distinctly hard and bulky on clenching indicates a muscular cause and a patient likely to respond.

A jaw that is wide skeletally will feel different, and treating it will not produce the change you are hoping for. Patients in that category are better served by surgical options or by contouring elsewhere in the face to change the overall proportion, and that assessment happens before anything is injected.

Many patients arrive for the facial change and stay for the relief. An enlarged masseter is usually enlarged because it is being overworked, which means the same patients frequently have jaw pain, morning headaches, and worn teeth from clenching and grinding.

What to Expect

Treatment takes roughly fifteen minutes and requires no downtime. Dosing is set by muscle size and strength, and this muscle is treated at higher doses than facial lines.

Slimming becomes visible at four to six weeks and deepens across repeated sessions, typically at three to four month intervals initially, with intervals lengthening as the muscle reduces. Most patients need several rounds before reaching their intended result, and it is worth knowing that going in.

Risks and Safety

Common effects include temporary bruising, tenderness, and reduced bite strength when eating tough foods during the first weeks.

Specific to this area, excessive dosing or imprecise placement can affect neighbouring muscles and produce temporary changes in smile or facial expression. Substantial masseter reduction also changes the midface, and in patients who are already thin through the cheeks the result can read as gaunt rather than refined. This is why dosing is built up across sessions rather than started at a maximum. Some patients notice jaw fatigue with prolonged chewing.

How long does jaw slimming take to work?

Longer than any other Botox® treatment, because the mechanism is muscle atrophy rather than muscle relaxation. Narrowing typically becomes visible at four to six weeks and continues developing over months and across repeated sessions. Patients expecting a result at two weeks are judging the wrong timeline. A realistic assessment point is after the second or third treatment.

Will jaw slimming work if my wide jaw is from bone?

No, and this is the single most important thing to establish beforehand. Botox® reduces muscle, and a jaw that is wide because of the underlying mandible will not narrow with it. That is why candidacy is assessed by palpating the muscle while you clench rather than by looking at a photograph. If your width is skeletal, you will be told so rather than sold a series of treatments that cannot deliver.

How long do jaw slimming results last?

Individual treatments last three to four months, but the cumulative effect is more durable than that suggests. A muscle repeatedly kept from full contraction reduces in size, and that reduction persists for a period even between treatments. Most patients find intervals lengthen over time, moving from three or four months initially to twice a year or less for maintenance.

Will jaw slimming affect my chewing?

Normal eating continues, since the treatment reduces contraction force rather than eliminating it. Some patients notice their jaw tires more easily with tough or chewy foods during the first weeks after treatment, which settles. At the doses used for slimming, which are higher than for facial lines, that effect is more noticeable than it is with TMJ treatment at conservative dosing.

Can jaw slimming make my face look gaunt?

It can, in patients who are already very thin through the midface, which is why dosing is built up across sessions rather than started high. Substantially reducing the masseter changes the whole lower face, and in a patient with little cheek volume the result can read as hollow rather than tapered. Where that risk exists, treatment is approached conservatively and sometimes combined with cheek volume to keep the proportion balanced.
