Vein Removal
The Nd Yag Laser is one of the best machines in the market available for reducing unsightly veins, usually in the legs. Repeated treatments are necessary and even then the success rate is about 60%. This low success rate can be improved by combining sclerotherapy and judicious surgical stripping of the veins with Nd Yag Laser.
Recurrence
Recurrence is common and the rate quite varied. This is because the spider veins are there inherently as a result of the pressure difference between the deep veins and the superficial set of veins. Blood flows in the path of least resistance. In the case of spider veins the blood flows from the deep set to the superficial set of veins. So in a typical course of treatments, the veins are destroyed after say 3-5 treatments. Depending on how much pressure difference there is between the deep and superficial set of veins, the spider veins might recur in one month or after 2 years.

Hair Removal
The complete destruction of Hair Follicles is nowadays believed to be brought about by the heat generated by melanin within hair. This happens when the light that falls onto the hair bulb (the bottom most part the hair follicle) is of a strong enough intensity. This pulse of light must also be of a short enough duration that it spares the surrounding tissues.
Growth Phases
Hair growth occurs in phases. Hairs can only be destroyed in their early anagen phase. In other phases, the hair bulb does not contain enough melanin to be effectively destroyed. Therefore several sessions over several months are required to remove about 80% of unwanted hair. How often the treatments are needed depends on which part of the body the hair grows from. Hair growth cycles vary on different areas of our bodies ranging from 2 weeks on the upper lips to 1 month in the bikini line.
A Modified Theory
Under the Modified Theory of Photothermolysis, the pit that houses the hair shaft has stem-cells that can regenerate into new hair bulb cells. These must also be damaged in order that the hair removal be permanent. Under this Theory, leaving a short shaft of hair facilitates the completeness of hair removal. Heat is generated along the hair shaft which then heats up the surrounding stem-cells along the hair follicle.
This causes a reddish goose-bump like appearance on the skin. It may be associated with a little itchiness and sometimes a pus-like reaction. It does not scar. But it is rather uncomfortable.
In Practice
In reality, burning not only the hair bulb but also the cells along the hair follicles poses practical problems. A lot of pain and discomfort often accompanies such degrees of destruction. In Asian skin, Post-Inflammatory Hyper-pigmentation (PIH) can also occur.
As a compromise, only the hair bulb is destroyed and some stem cells along the hair shaft survives and later produces small tufts of fine hairs again, although at a much slower rate.
